Triumph Partners with Greater Health Now's Community Hub!

Greater Health Now’s Community Hub connects individuals to health-related social needs services and public benefits. The Hub brings together community-based organizations, behavioral health providers, and healthcare partners to deliver culturally responsive care coordination tailored to community members’ needs. Triumph is now partnering with Greater Health Now’s Community Hub, expanding access to coordinated, whole-person support for individuals and families. All Community Hub services and benefits are provided at no cost to those served.
